Abstract
Background The existing hospice advance care planning (ACP) resource for patients was clinically focused, not inclusive and dated. Both clinical and community engagement teams requested a more comprehensive resource.
Aim To update and expand the existing ACP resource for patients including:
Improved structure and formatting.
More inclusive and comprehensive content.
Accompanying workshops/event templates to encourage conversations about planning for the end of life.
Method We reviewed a range of ACP and funeral/death planning resources identifying important elements for inclusion. We consulted with staff ensuring emotional, spiritual, psychosocial and practical needs and options were incorporated. We consulted stakeholders including the bereaved, hospice patients, and external networks to ensure practical usability and inclusivity. We redesigned the community engagement ACP workshop to align with the new resource and trialled it with hospice staff to fine-tune before promoting it to community groups. We ensured the new resource was accessible for completion both online and in print. We added a discussion-based event to our community engagement toolkit to share the new resource and the benefits of ACP - the ‘Power of Planning Ahead Café’. We created a promotional plan across Jan-June 2024.
Results 600 copies of the resource distributed. Our focus for Dying Matters Week 2024 was ‘the power of planning ahead’. We ran our new ‘Power of Planning Ahead Café’. 100% of attendees agreed or strongly agreed that the event ‘helped me to understand the choices available to me at the end of life’.
We distributed the new resource at: an internal staff pop-up stand; two library open days; two hospital fairs; one hospice open evening and two GP clinics.

Conclusion Promotion of the booklet continues. Plans for more café events and training are in plan into 2025.
