Abstract
Background Hospices have limited Occupational Therapy (OT) resource and funding models influence services delivered. The occupational therapy workforce face significant challenges. The role of Trusted Assessor (TA) improves access to timely assessment for equipment in homes facilitating timely discharge from hospice reducing delays in care. Thus potential to promote independence, reduce dependence on families, hospices and health and social providers. Also reduction to costs at a system level.
Aim Enhance timely discharges from ward hospice setting, support individuals, and their loved ones in the community.
Methods Review and restructure of allied health professionals service model of provision performed. Alongside national trends, a gap in OT provision for 18 months meant reliance on over-stretched community services and provision gaps. Reviewing literature and national guidance identified the role of Trusted Assessor and training pathway. Data retrospectively collated between January 2023-January 2024 (electronic patient information system) identified all interventions of TA. Activities include home, ward and telephone assessments, equipment delivery, set-up, and review.
Real-time feedback from service users as quotations.
Results Two team members qualified, roles implemented Jan 2023. During OT recruitment gap 333 activities undertaken by TA January 2023 – January 2024. Without TA, provision would not have been achieved leading to referral to external services, with anticipated delays.

Conclusion Highly successful project shown by the number of activities during the recruitment gap. Continuation in the service to enhance OT provision and continued increase of service provision for individuals supported by the OT service.
Plans Extend the training/role to the wider support workforce with potential to strengthen equity of service availability, sustainability of future services via adaptable approach continuing to provide quality care.
