51 Observational study of the prevalence of hospital inpatients in the last year of life

Abstract

Background A third of hospital adult in patients are in their last year of life (Clarke et al. 2014) and almost 50% of people die in hospital. At Dudley Group NHS Foundation Trust (DGFT) we have implemented the Gold Standards Framework (GSF) and we are able to record the GSF status on our hospital electronic patient record. From this we produce a monthly report with regards to several metrics including% of patients identified as GSF and the repeat observational study was carried out to provide evidence for the wards when reviewing their metrics as each ward will have a different percentage of patients in the last year of life.

Method The inpatients on adult wards were identified on the 1st April 2022 through the information team and the number of deaths in the following year recorded in order to review the% of adult inpatients in the last year of life within the hospital and broken down by ward.

Results Retrospectively across DGFT the percentage of inpatients in the last year of life on 01/04/22 was 34% with variability across ward areas from 61% to 5%. This is fourth time this snapshot review has been completed and in each year the percentage of adult inpatients in the last year of life has been approximately 30%, except for 2020 which was 40%.

Conclusion The prevalence of inpatients in the last year of life from the snapshot remains similar across the trust compared to previous reviews. The wards and divisions use this data together with the metrics including identification of GSF to support setting targets and understand their patient population. Identification allows the implementation of individual plans of care for patients and this work supports the beneficial role GSF implementation has on patient identification.
